Privacy Policy

Serena’s Art Therapy is a practice for psychotherapy working with children and adults. (KVK 90094121). 

 

In this privacy policy I inform you on the what personal information will be collected and processed. 

 

1. Personal data: all information with which directly and indirectly would identify a person: name, ID, medical data and location and more.

 

1. Processing

Any personal data will be stored and collected according to ABG and WGBO. Serena  Ha, the founder of Serena’s Art Therapy will be acting as the processor.  

 

These includes: 

 

3.1 Patient Care

Your personal data for financial administration (invoicing) and medical record keeping and client’s notes (therapeutic process) and referral and consent and artwork (made in duration of your therapy). You will be asked to bring one or two piece of ID at the start of therapy for verification. 

 

3.3 Website Visit

Serena’s Art Therapy’s website will collect your information you shared, for example, when you filled the contact forms. Also, the website may record certain information like our location, time, and day you visited the website. 

 

4.1 Provide assessment/therapy services and related activities

As mentioned your personal data for referral, consent forms, other related client files will be collected and stored confidentiality unless it is agreed upon both parties to share with third parties: GP (general practitioners), authorities, schools, hospitals and other required and necessary parties (parents for children, social workers and care workers). 

 

4.2 Complying with legal obligations 

Serena’s Art Therapy may be obligated to comply to legal obligations: 

• Record keeping of clients 

• Quality standards and audits for Dutch’s Visitatie-BV and UK’s HCPC

• Exchange with insurers and municipalities (if required you will be informed)

• Child Protection policy (Dutch: ‘Meldcode Kindermishandeling’) and incident (happened to clients during therapy I will report it), safeguarding (self-harm and suicidal)

 

5. Your permission

It is important you read and meet Serena Ha for an initial consultation before you decide to start therapy so you are informed and comfortable to start therapy. Therapy does not start if you do not consent and give permission to shared your personal data on referral and consent forms. Also, there is a parental consent form for parent/guardian to give permission for Serena Ha to work with any children (under 18 years old). 

 

6. Security 

All of your personal data when you agreed to start therapy will be stored in a digital format with encrypted password and your physical artwork will be stored in a locked cabinet in Serena’s Art Therapy’s office. 

 

7. Retention Period

Serena’s Art Therapy will store all clients files up to 20 years and 20 years starts for children are 18 years old. If something happened to the founder, Serena of Serena’s Art Therapy, there is a clinical will created with a substitute art therapist to maintain client’s files storage. 

 

8. Data protection and integrity 

Serena’s Art Therapy will report to Dutch Data Protection services to notify of any data breach or loss.
